OMG Girls Club Application

Parent/Guardian Consent:

By submitting this application, I confirm that I am the parent or legal guardian of the creator listed above and give permission for them to apply to and participate in the OMG Girls Club creator program. I am singing off as the official program participant/contact who will be the one in communication with OMG Accessories. 

I understand that, if selected, my child may receive gifted merchandise from OMG Accessories and may be asked to create and post content featuring OMG Accessories products on their social media account(s).

I give OMG Accessories permission to share, repost and feature approved photos and videos created as part of the OMG Girls Club program that include my child and/or OMG Accessories products on OMG Accessories’ social media accounts &/or website.

I understand that specific content requirements, posting guidelines, deadlines, usage permissions or additional terms associated with individual collaborations will be provided to me for review when applicable.

In order to stay in the program, the creator must produce the required content each month. If content requirements are not met, they will not qualify for the next collection package. 

By submitting this application, I acknowledge and agree to the permissions outlined above.
